Adoption Application

If you would like to adopt one of our dogs please complete the application below or download one here and we will get back to you as soon as possible.

Please note that this application is the first step to adopting one of our dogs; after meeting the dog should you decide to adopt we do require a simple home check. The home check is to assure us that you can provide a safe home environment for the dog and that all family members are willing to adopt.

In order to be considered as an adopter you must:

Be 18 years of age or older.
Have identification showing your present address.
Have the knowledge and consent of your landlord, if applicable.
Be able and willing to spend the time and money necessary to provide training,
medical treatment and proper care for a pet.
All current dogs and cats in your residence MUST be spayed/neutered, if they are not your application will be denied.

Fuzzy Pawz Rescue reserves the right to deny any applications that we deem unfit or unsuitable for the animal. Please be aware that our animal adoptions are NOT on a first come- first served basis. Our sole purpose is to find the best possible homes for the animals that fall under our care. If the representative determines that your home is not the BEST fit for the dog you have applied for, you may be contacted regarding other dogs that more closely match your interests, life style, and family environment, or referred to a different adoption agency. Our goal is to find the right home for the right dog; people who are willing to provide permanent loving homes. We want every match to be the perfect match not the quickest match. Please make sure you have considered carefully the time and money required to own a pet before you make the decision to adopt. Owning a pet is a lifetime commitment.

WE spay/neuter and vaccinate all of the dogs that we place into foster care, so your newly adopted dog is ready to go!

Completion of this application does not guarantee adoption of a Fuzzy Pawz Rescue dog.
